**What to Expect (Job Responsibilities)**
As a Field Community Health & Social Impact Specialist, you will play a critical role in ensuring the quality and effectiveness of our healthcare services. Your responsibilities will include:
* Conducting medical management activities to ensure quality patient care
* Collaborating with healthcare providers to coordinate and manage patient care
* Reviewing medical records and treatment plans to ensure compliance with regulations
* Providing guidance and support to patients and their families regarding medical treatment options

**What is Required (Qualifications)**
To be successful in this role, you will need to possess the following qualifications:
* Bachelor's degree in Nursing or related field
* Active RN license in the state of Maryland
* Minimum of 3 years of experience in medical management or healthcare coordination
* Strong knowledge of medical terminology and procedures
* Excellent communication and interpersonal skills

* Certification in Case Management (CCM)
