Job Summary
As a Telemedicine Registered Nurse with our team, you'll play a vital role in delivering high-quality, patient-centered care via phone and chat. You'll work closely with our interdisciplinary team to provide clinical triage, post-discharge follow-ups, and care coordination services, with a focus on medical weight loss and GLP-1 management. Key Responsibilities
- Conduct phone-based clinical triage and assessment of patients, utilizing your expertise to identify and prioritize their needs.
- Provide post-discharge follow-up care and support to patients, ensuring a smooth transition and promoting optimal health outcomes.
- Coordinate care with patients, families, and healthcare providers, leveraging your knowledge of medical weight loss and GLP-1 medications to inform care decisions.
- Utilize electronic health records (EHRs) and other telehealth tools to document patient interactions, track progress, and communicate with care teams.
- Collaborate with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.
- Stay up-to-date with the latest developments in medical weight loss and GLP-1 management, applying this knowledge to inform your practice and drive continuous improvement.
Requirements and Qualifications
Essential Qualifications
- Active and unencumbered compact RN license required, with a minimum of 3 years of experience in clinical settings such as telemedicine, acute care, or case management.
- Clinical background in medical weight loss and GLP-1 medications is essential, with a deep understanding of the complexities and nuances of these areas.
- Technology skills are critical, with the ability to comfortably use multiple platforms (e.g., EHR, Slack, telehealth tools) simultaneously.
- Availability of at least 20 hours per week, with daytime, evening, and weekend availability required to meet the needs of our patients.
Preferred Qualifications
- Prior telemedicine experience is highly valued, as it demonstrates your ability to navigate the unique challenges and opportunities of virtual care.
- Fluency in Spanish is an asset, as it enables you to communicate effectively with a broader range of patients and families.
