Job Summary
As a Remote Licensed Practical Nurse - Care Coordinator, you will play a vital role in managing patient care and coordinating resources to ensure seamless transitions and optimal health outcomes. You will work independently in a remote setting, utilizing your clinical expertise and strong communication skills to educate patients, manage medications, and resolve patient concerns. Key Responsibilities
- Patient Care Management: Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Resource Coordination: Identify and coordinate community resources with patients that would benefit their care.
- Patient Education: Provide patient education and health literacy on the management of chronic conditions.
- Medication Management: Perform medication management, including identifying potential medication concerns, reconciliation, adherence, and coordinating refills.
- Service Delivery: Assist in ensuring timely delivery of services to patients, including Home Health, DME, Home Infusion, and other critical needs.
- Patient Communication: Resolve patients' questions and create an open dialogue to understand needs.
- Referral Management: Assist/manage referrals and appointment scheduling.
Requirements
Essential Qualifications
- Licensure: Active Multi-State/Compact License (LPN) (NLC) (LVN).
- Experience: At least 3 years of direct patient-facing work experience as an LPN/LVN.
- Technical Aptitude: Proficiency in Microsoft Office Suite.
- Communication Skills: Excellent written and verbal communication skills.
Preferred Qualifications
- Epic Experience: Experience working with Epic electronic health records.
- Bilingual: Proficiency in multiple languages.
- Additional Licensures: Additional single-state licensures (LPN).
Remote Work Requirements
- High-Speed Wi-Fi: Must have active high-speed Wi-Fi.
- Home Office: Must have a home office or HIPAA-compliant workspace.
Physical Requirements
- Sedentary Work: This position is sedentary and will require sitting for long periods of time.
- Communication: The position will require the ability to speak clearly and listen attentively, often by telephone, for an extended period of time.
- Note-Taking: The position will require the ability to understand, process, and take thorough notes in real-time on telephone conversations.
