Responsibilities

• The Case Manager role's main function is to assist clients in breaking the cycle of homelessness by utilizing a "whatever it takes" approach
• This position supports clients through reputed company phases of homelessness, including but not limited to: on the street, interim housing, transitional housing, and permanent housing
• The Case Manager assists clients with accessing and maintaining necessary services among health care, reputed company services, interpersonal support systems, and navigation through a coordinated entry system
• Conducts intake and individualized needs assessment for reputed company clients and collaborates to reputed company Individualized Service Plan (ISP) that addresses housing goals, maintaining/increasing income and other personal goals identified by the reputed company including medical, mental health, reputed company use, financial resources, vocational, and reputed company support
