Experienced Part Time Nurse Practitioner – Home-Based Medical Care Specialist for Complex, Chronic Patients in Chesterfield County, SC

Position Highlights and Primary Responsibilities
As a valued member of our Landmark team, you will be responsible for establishing and maintaining effective relationships with patients, caregivers, and/or their legal representatives, as well as collaborating with interdisciplinary team members, primary care physicians, and other healthcare providers. Your primary responsibilities will include:
- Conducting initial comprehensive assessments on newly enrolled patients and providing ongoing care, including visits to address changes in condition
- Developing and updating patient management care plans to ensure the highest standards of care and promote the improvement of care management and delivery
- Consulting with Physician Leaders and coordinating with primary care physicians to ensure seamless care coordination
- Performing basic procedures of episodic nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, and basic wound care, with new hire and annual skills training provided
- Participating in HEDIS measure campaigns and other quality initiatives to drive excellence in care delivery
- Participating in clinical after-hours on-call programs on a rotational basis to ensure continuous patient support
Required Qualifications
To be successful in this role, you will need to possess the following essential qualifications:
- Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
- Current, unrestricted NP License in the State of Practice
- Active, unrestricted RN licensure in the State of Practice
- Access to reliable transportation, with compliance with all terms of the Optum Motor Vehicle Safety policy if driving a vehicle
Preferred Qualifications
While not required, the following preferred qualifications will make you a more competitive candidate:
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar settings
- Proven knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
- Proven knowledge of CPT, ICD-10, and HCPC codes
- Proven knowledge of clinical standards of care and demonstrated awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
Skills and Competencies
To excel in this role, you will need to possess excellent communication and interpersonal skills, with the ability to build strong relationships with patients, caregivers, and healthcare providers. You should also be comfortable working in a fast-paced, dynamic environment and be able to adapt to changing patient needs and priorities.
