Job Overview and Primary Responsibilities

As a Part Time Nurse Practitioner in our Home-based Medical Care team, you will play a vital role in bringing medical care to complex, chronic patients in the comfort of their own homes. This life-changing work helps give older adults more days at home, and we are seeking a skilled and compassionate professional like you to join our team. Your primary responsibilities will include:

  • Establishing and maintaining effective relationships with patients, caregivers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
  • Performing an initial comprehensive assessment on all newly enrolled patients and providing ongoing care thereafter, including visits to address changes in condition
  • Developing a patient management care plan upon enrollment and updating it as needed when changes in condition warrant or following hospitalization
  • Consulting with Physician Leaders and coordinating with PCP in these decisions as available
  • Performing basic procedures of episodic nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, basic wound care, etc., with new hire and annual skills training provided
  • Participating in HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
  • Participating in clinical after-hours on-call program on a rotational basis

Essential Qualifications and Skills

To be successful in this role, you will need to possess the following essential qualifications and skills:

  • Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Current, unrestricted NP licensure in the State of Practice
  • Nurse Practitioner must actively hold, unrestricted RN licensure in the State of Practice
  • Access to reliable transportation, with compliance with all terms of the Optum Motor Vehicle Safety policy if driving a vehicle
  • Strong communication and interpersonal skills, with the ability to build effective relationships with patients, caregivers, and healthcare professionals
  • Ability to work independently and as part of an interdisciplinary team, with a strong focus on patient-centered care
  • Comfortable with basic procedures of episodic nursing care, with a willingness to learn and develop new skills

Preferred Qualifications and Experience

While not essential, the following preferred qualifications and experience will be highly regarded:

  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar setting
  • Proven knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
  • Proven knowledge of CPT, ICD-10, and HCPC codes, with a strong understanding of clinical standards of care
  • Demonstrated awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
  • Experience working with electronic medical records and other healthcare technology systems
