Job Summary

As a Pharmacy Care Coordinator, you will work closely with clinical pharmacists, healthcare providers, and internal departments to coordinate patient care and treatment plans. You will utilize your pharmacy expertise and customer service skills to ensure seamless communication and collaboration with patients, providers, and internal stakeholders. This role offers a hybrid work model, combining remote work with occasional time in our office.

Key Responsibilities:

  • Consult with and coordinate ongoing care and treatment plans for members with clinical pharmacists and other providers.
  • Document pertinent patient information in the electronic medical record.
  • Assist with data management and recording results for month-end reports.
  • Participate in Pharmacy Services wellness programs, including transition of care medication reconciliation.
  • Retrieve pertinent patient information for new patient referrals or recent hospital/SNF discharge.
  • Contact patients post-hospitalization to perform medication review and information gathering.
  • Perform patient follow-up procedures.
  • Answer incoming phone calls and direct patient messages.
  • Coordinate with other healthcare providers, healthcare professionals, and healthcare team members.
  • Update job knowledge by participating in educational opportunities and reading professional publications.
  • Participate in process improvement initiatives, staff meetings, and departmental meetings.
  • Use, protect, and disclose Optum patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
  • Adhere to Universal Precautions and safety policies and procedures at all times.

Requirements and Qualifications

Essential Qualifications:

  • High School Diploma / GED (or higher).
  • Possess and maintain an active pharmacy technician license in the state of Oregon.
  • 1+ year of experience as a pharmacy technician.
  • 1+ year of experience in customer service.
  • 1+ year of experience with medical and pharmacy terminology.
  • 1+ year of experience with medication therapeutic categories.
  • 1+ year of experience with identifying generic names for brand name drugs and vice versa.
  • Basic level of proficiency in Windows and MS applications (Word, Excel).

Preferred Qualifications:

  • National Pharmacy Technician Certification.
  • Possess and maintain an active pharmacy technician license in the state of Washington.
  • 3+ years of experience working as a pharmacy technician.
  • 1+ year of experience in a hospital, ambulatory care, or other healthcare setting.
  • Experience in a managed care setting.
  • Bilingual (Spanish/English).
