The Supervisor, Post-Acute Complex Care (RN) is a multi-market leadership position accountable for the direction, leadership and support of our Post-Acute Care Coordinators and Post Acute Manager, Complex Care in delivering the day-to-day programmatic acute care strategy. The incumbent is responsible for clinical oversight of our care coordination efforts across the continuum of care, ensuring that our patients receive and experience the highest quality of care at post-acute facilities, leveraging all available resources, in line with the ChenMed model and Complex Care Team standards of care. The Supervisor, Post Acute Complex Care (RN) reports to the Manager, Post-Acute Complex Care and collaborates closely with Acute and Post-Acute Care leadership to ensure that the clinical needs of our patients are met in all facility-based healthcare environments (post-acute care) with high quality care and proper utilization of healthcare resources. This role collaborates closely with facility-based healthcare providers (post-acute care providers, case, and utilization management departments), strategic payor partners and our PCP-led care teams to ensure the patient's progression through the continuum of care is effective, efficient, equitable, safe and achieves the desired clinical outcomes with effective resource utilization. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Supervises a team of Post-Acute Care Coordinators and Post Acute Managers, Complex Care in 1-2 markets. Provides day-to-day oversight of staffing/scheduling needs, problem solving, and performance management. Participates in recruitment process and talent management activities. Conducts regular coaching and mentoring sessions. Supports the Complex Care and Clinical Strategy Teams in the day-to-day execution of our post-acute care standard operating procedures and ensures Post Acute Care Coordinators and Post Acute Care Managers deliver on these standards. Monitors performance and delivery of Complex Care key performance indicators related to post-acute care management at the local level. Educates post-acute care coordinators and Post Acute Care Managers on appropriate levels of care in the post-acute setting (HHA, SNF, IRF, LTAC) in accordance with industry standards and federal and local guidelines. Ensures post-acute care coordinators and managers understand key performance indicators and have necessary tools and resources to effectively deliver expected outcomes. Coaches the post-acute care team on identifying and managing safety risk, functional status assessment, medication management, and knowledge deficit identification. Implements the post acute coaching program at the market level. Ensures team members execute length of stay mitigation that results in safe and effective discharge processes. Ensures team members can identify barriers for safe discharge and execute standard operating procedures to achieve readmission avoidance. Provides direct support for care coordination of complex patients in post-acute care facilities in collaboration with post-acute physicians and management. Seeks assistance from ChenMed's Specialty and Network Teams when needed to support patient care. Assists with implementation of SOPs as requested by management. Participates in quality improvement initiatives and special projects as assigned. Provides coverage for PTO and assists with staffing needs as requested. Serves as a preceptor when assigned. Ensures compliance with all operating processes and service excellence standards. Assists with quality audits and identifies opportunities for improvement. Supports Event Notification System (ENS) alert management and data capture processes. Performs other duties as assigned. KNOWLEDGE, SKILLS AND ABILITIES: Strong interpersonal and communication skills and ability to work effectively with internal and external stakeholders Critical thinking skills and ability to work with moderate autonomy Leadership skills to guide team members toward objectives Ability to manage multiple priorities and work in stressful situations Commitment to maintaining confidentiality and patient care standards Flexibility with schedule, including off-shifts, weekends and holidays Knowledge of care coordination standard operating procedures Understanding of case and utilization management procedures in post-acute settings Basic understanding of healthcare economics and Medicare cost drivers Understanding of Managed Medicare principles Ability to assess patient progress and adjust care plans Knowledge of nursing and case management theory and practice Knowledge of community health services and social service networks Organizing and coordinating skills Proficiency in Microsoft Office Suite products including Excel, Word, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software Ability to travel locally up to 20% of the time Spoken and written fluency in English, bilingual preferred Requires use of independent judgment EDUCATION AND EXPERIENCE CRITERIA: BA/BS degree in Healthcare Administration or related field preferred A valid, active Registered Nurse (RN) license in State of employment plus one of the following: Bachelor's Degree in Nursing (BSN); OR Associate degree in Nursing (ADN) with 3 years of recent experience in acute or post-acute healthcare facility OR Bachelor's degree in health-related discipline Compact license required if available in state Three (3) years of recent experience in acute or post-acute healthcare facility Minimum three (3) years of care management experience One to two (1-2) years of supervisory/lead experience in acute care, post-acute care or care coordination setting preferred Basic Life Support (BLS) certification required within first 90 days Current, valid Driver's License required Training in operational and quality improvement skills preferred Knowledge of managed care environment required PAY RANGE: $81,202 - $116,002 Salary EMPLOYEE BENEFITS https://chenmed.makeityoursource.com/helpful-documents We’re ChenMed and we’re transforming healthcare for seniors and changing America’s healthcare for the better.
