Job Summary
We are seeking a highly skilled and compassionate Medical Social Worker (MSW) to join our team in the Greater Oakland County Area, Michigan. As a Home Health and Hospice MSW, you will provide home-based social services to patients dealing with depression, anxiety, poor nutrition, or a lack of resources that make managing their injury or disease difficult. You will work as part of a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, PTs, OTs, and SLPs, to develop and implement patient care plans that address their unique needs and goals.
Key Responsibilities
- Provide home-based social services to patients with various medical conditions and psychosocial needs
- Conduct patient assessments, document progress, and ensure patients are moving toward their goals
- Develop and implement patient care plans that address their unique needs and goals
- Provide counseling, community resource planning, crisis intervention, and advocacy
- Collaborate with family and caregivers on patient care plans and provide education on how to support patient progress
- Monitor patient psychosocial conditions and identify social and emotional needs
- Consult with other healthcare professionals to ensure comprehensive care
Requirements
- Education: Master's degree in Social Work (MSW) from an accredited program
- Licensure: Active MSW license in the state of Michigan
- Experience: Minimum 1 year of social work experience, ideally in home healthcare
- Knowledge: Familiarity with state and federal home health regulations
- Skills: Strong interpersonal skills, good communication, empathy, compassion, and resourcefulness
Preferred Qualifications
- Experience working with patients with complex medical conditions
- Knowledge of community resources and services
- Certification in a related field (e.g., LCSW, LSSW)
