Job Summary

As an Occupational Therapist Assistant - Home Health PRN, you will provide skilled occupational therapy services on an intermittent basis to clients in their homes under the supervision of an Occupational Therapist. You will play a vital role in helping patients achieve independence and improve their physical and mental functioning.

Key Responsibilities:

  • Provide client care services delegated by the Occupational Therapist, including evaluation, development of rehabilitative plans of care, and periodic re-evaluation.
  • Assist in the preparation of clinical and daily progress notes, and submit them according to company policy.
  • Instruct clients and their families in exercise programs, therapeutic self-care, and creative activities to improve independence and physical and mental functioning.
  • Identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication, and report them to the Occupational Therapist.
  • Observe, record, and report clients' reactions to treatment and any changes in their condition.
  • Consult with the Occupational Therapist regarding the outcome of home visits and report physical findings.
  • Assist in the evaluation and development of rehabilitative plans of care, and participate in periodic re-evaluation/reassessment as indicated.
  • Instruct other health team personnel, including Home Health Aides and family members, in certain phases of occupational therapy.
  • Write daily progress notes on patients, and submit them according to company policy.
  • Attend rehabilitation department staff meetings and case conferences as required.
  • Instruct clients in the care and use of wheelchairs, braces, splints, and prosthetic and orthotic devices.
  • Participate in discharge activities and assist the Occupational Therapist in completing the Occupational Therapy Discharge Summary.

Qualifications:

  • Graduate of an accredited occupational therapy assistant program.
  • Current certification or license as required by state law.
  • Current BLS Certification.
  • Six (6) months to one (1) year of Home Health experience preferred.
  • Excellent verbal, written, and interpersonal communication skills.
  • Results-oriented with strong critical thinking and problem-solving skills.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
