About the position
Responsibilities
• Develop a proactive plan of care to enhance short and long-term outcomes for members.
• Conduct evaluations of member needs using clinical tools and data review.
• Incorporate strategies to reduce risk factors and address complex health and social indicators.
• Perform assessments considering various information sources, including claims data.
• Utilize a holistic approach to determine the need for referrals to clinical resources and interdisciplinary team members.
• Collaborate with supervisors and stakeholders to overcome barriers in achieving member goals.
• Present cases at interdisciplinary case conferences.
• Utilize motivational interviewing skills to engage members and assess their health status.
Requirements
• Minimum 3+ years of clinical practical experience.
• Minimum 2+ years of case management, discharge planning, or home health care coordination experience.
• Active and unrestricted RN licensure in the state of Texas.
