We are seeking an experienced Licensed Practical Nurse (LPN) to join our team at GSS MN Westbrook Ctr, a small facility located in Westbrook, MN, where you can make a real difference in the lives of our residents.

Job Summary

We're seeking an experienced LPN to provide professional nursing care to our residents in a long term care setting. As an LPN with our team, you'll work under the supervision of a registered nurse, advanced practice provider, or physician to deliver high-quality care and services to our residents. You'll be responsible for assessing residents, administering medications, and providing technical support to healthcare professionals as needed. You'll also be expected to document resident interactions and outcomes, and to work effectively with our interdisciplinary care team to ensure the best possible outcomes for our residents.

Key Responsibilities

  • Provide professional nursing care to residents of all ages in a long term care setting
  • Assess residents and administer medications or treatments as ordered
  • Provide technical support to healthcare professionals as needed
  • Document resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary
  • Establish and maintain effective working relationships with residents, healthcare providers, and the public
  • Communicate clearly and maintain confidentiality of sensitive information
  • Counsel residents, family members, and/or resident advocates on prevention and treatment plans under the direction of a registered nurse, advanced practice provider, or physician
  • Develop self-care programs for residents using resident education principles
  • Perform other related duties as assigned

Qualifications and Requirements

To be considered for this role, you must meet the following qualifications:

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse
  • Associate degree in practical nursing is preferred
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges
  • Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum, or have had a minimum of 400 hours of experience as LPN at another facility
  • Specific services/positions may have additional education, training, or experience requirements

Preferred Qualifications

  • Experience working in a long term care setting
  • Previous experience working with seniors or other vulnerable populations
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment

What We Offer

As a member of our team, you'll enjoy a competitive salary range of $27.00 - $36.00 per hour, depending on your licensed years of experience. You'll also receive:

  • Additional $2.50/hr straight night differential + $1.25/hr weekend differential
  • 12-hour Night Shifts: 6:00 p.m. - 6:00 a.m.
  • Scheduled 2 shifts per week, including weekend and holiday shifts
