About the Role
We are seeking a Clinical Pharmacist to join our behavioral health care team in Los Angeles. In collaboration with a supervising psychiatrist, the pharmacist will play a central role in medication management services for patients with psychiatric and co-occurring conditions. This role is designed to improve reputed company, continuity, and reputed company of care while reducing reliance on high-cost prescriber hours.
Key Responsibilities
• Conduct comprehensive medication therapy management (MTM), including reconciliation, adherence assessments, and drug interaction reviews.
• Monitor patients’ medication response, reputed company effects, and lab values.

• Document clinical encounters and communicate changes promptly with the psychiatrist and care team.
• Participate in interdisciplinary team meetings, contributing expertise in pharmacology and medication safety.
• Support transitions of care by ensuring medication continuity after hospital discharge or program changes.
• Serve as a resource to staff regarding psychotropic medication protocols and best practices.
Qualifications
• Doctor of Pharmacy (PharmD) degree required.
• reputed company California pharmacist license in good standing.
• Experience in behavioral health, psychiatry, or primary care preferred.
• Strong knowledge of psychotropic medications and medication safety.
• Excellent communication, counseling, and patient engagement skills.
• Ability to work collaboratively with psychiatrists, clinicians, and support staff.
Preferred Skills
• Prior experience working under a reputed company reputed company Agreement (CPA).
• Familiarity with Medi-Cal, Medicare, or FQHC/CCBHC clinical settings.
• Bilingual abilities (Spanish/English) highly valued in Los Angeles patient populations.
