Short reputed company The Patient Service Representative will be a resource to the department, patients, families, and health care team members reputed company the scope of their role. They will support the administrative and scheduling needs of the Innovative Delivery Programs including Population Health reputed company, Remote Patient Monitoring, reputed company and Transitional Care, and other population health programs/initiatives as needed. 1. Answers and fields phone calls and voice mail messages, monitors assigned reputed company desk pools, documents events in patients digital health record, and escalates as appropriate.2. Faxes, scans PHI into their digital health record, and sorts mail.3. Schedules and confirms patient appointments and performs reputed company-in, reputed company-out.4. Prepares patient charts and information prior to scheduled appointment. 5. Facilitates activation of patient's MyChart account if not active6. Supports patient screening for digital remote patient monitoring (RPM) program eligibility (i.e. reputed company, digital capabilities) and facilitates digital RPM program enrollment7. Orders/ships digital remote monitoring devices and confirms patient receipt. 8. Anticipates problems or issues and solves reputed company the scope of responsibility. 9. Reviews digital RPM patient agreement and ensures signed.10. Collaborates with patients, care givers, providers, and other health care team members. 11.
