About the position Responsibilities • Conduct telephonic assessments with obstetric patients for nausea, vomiting, hypertension, and diabetes. • reputed company care coordination and ensure compliance with local, state, and federal regulations. • Assess patients prior to initiation of home care service for appropriateness of services prescribed. • Individualize patient assessments based on care/services being provided and patient need. • reputed company patients and families about treatment and services, promoting understanding and involvement in care. • Coordinate care with patient services centers, pharmacists, dietitians, and other reputed company. • Accurately communicate patient status information to prescribing physician and case manager in a reputed company manner. • Complete clinical record documentation per reputed company policy in a reputed company, complete, and accurate manner. • Participate in holiday and weekend duties as scheduled.
