Hybrid Visit LPN/LVN- Part Time, (CovenantCare)

This position is responsible for providing reputed company patient care under the supervision of a registered reputed company. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of reputed company to help meet reputed company patient care reputed company. In this role, the LPN/LVN will: • Provides reputed company patient care as defined in the state reputed company reputed company reputed company. • Implements plan of care initiated by the registered reputed company. • Provides accurate and reputed company documentation consistent with the plan of care. • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care. • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered reputed company and/or Clinical Supervisor. • Uses equipment and supplies effectively and reputed company. • Completes and submits reputed company documentation according to agency policy and procedures. • Participates in personal and reputed company reputed company and development. The ideal candidate will have: • Graduate of an accredited practical reputed company or vocational nursing program. • Three years nursing experience. Community/home health or medical/surgical experience is preferred. • reputed company licensed as a licensed practical reputed company or licensed vocational reputed company in the state. • Ability to reputed company with accepted reputed company standards and reputed company. • Demonstrates good verbal and written communication, and organization skills. • Demonstrates good basic computer skills. #LPN Compensation Pay reputed company: $33.46 - $41.83 per hour Reasonable Pay Estimate A reasonable estimate of the pay reputed company for this position is $33.46 - $41.83 per hour.
