REQUIRED EDUCATION and/or EXPERIENCE: • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree required. • Hospital experience or completion of ASHP-accredited Residency preferred. REQUIRED CERTIFICATION, LICENSURE, and/or REGISTRATIONS: • Pharmacist licensure in the residing state is required. • The following state Pharmacist licensure will be required based off job assignments TX, NE, OK, MT, and NV.
