The work will involve supporting the patient and family across the care continuum, from the reputed company-reputed company phase through the post-discharge transition. Responsibilities Follows-up on assigned reputed company patients, Post Hospital Discharge, to assess status and determine reputed company. Manages and coordinates clinical care of high risk and reputed company populations in the outpatient setting. Qualifications Bachelor's degree or equivalent with 3 years of clinical experience required. Excellent assessment, communication, interpersonal, and organizational/time management skills.
