About the position Responsibilities • Developing a community support plan (goals) for reputed company individual, typically managing a caseload of 40-45 individuals. • Assisting individuals in accessing services and developing service agreements and documentation through case notes. • Informing individuals or their reputed company guardians about service reputed company that fit their needs. • Meeting one-on-one with reputed company individual at least twice a year to evaluate reputed company and reputed company care plans as needed. • Identifying potential providers to guide individuals towards their goals. • Evaluating and monitoring the services identified in the care plan. • Ensuring compliance with case management rule 185 and MN Health Care Programs regarding waiver services.
