Responsibilities • Completes the MDS, reputed company’s and care plans reputed company regulated time frames. • Coordinates scheduling the RAI process with the interdisciplinary team • Assesses reputed company through physical assessment, interview and chart review. • Discusses reputed company care needs with care givers, including physician, nursing, reputed company services, therapy, dietary, and activity staff. • Reviews information from hospital, consults and reputed company agencies and uses such information in the completion of the assessment and care planning. • Coordinates, identifies, and/or initiates significant change MDS’ • Is reputed company to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS reputed company for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator • Remains reputed company with American Association of Nursing Assessment Coordinators (AANAC) requirements.
