We are seeking an experienced and compassionate Case Manager to join our team on a part-time basis. As a Case Manager, you will work closely with our Program Manager to ensure the effective delivery of services to our veterans. Your primary responsibilities will include conducting home visits, managing veteran files, and collaborating with financial services to ensure seamless program operations.Key Responsibilities
- Deliver engaging training sessions tailored for veterans and their representatives, promoting empowerment and self-sufficiency.
- Conduct quarterly home visits for assigned veterans to assess and meet their needs, fostering a supportive and inclusive environment.
- Oversee the intake process for new VD-HCBS referrals, ensuring timely submission of enrollment paperwork to the appropriate financial management services and the Department of Veterans Affairs.
- Maintain thorough documentation of all correspondence regarding VD-HCBS referrals while managing comprehensive veteran files, adhering to confidentiality and security regulations.
- Review and ensure the accuracy of veterans' timesheets, making necessary corrections and updates in the electronic database.
- Develop and evaluate budget plans for veterans to guarantee precise financial reporting, promoting fiscal responsibility and transparency.
- Assess veterans' needs to confirm the appropriateness and effectiveness of services provided, identifying areas for improvement and implementing corrective action plans as needed.
- Promptly communicate with the Program Manager and financial services regarding any veteran's institutional placements, ensuring seamless program operations.
- Collaborate with the Program Manager on new enrollments, changes in veteran status, and issue resolutions, promoting a culture of teamwork and collaboration.
- Address and manage any veteran-related issues in partnership with the Program Manager, assisting in the approval of necessary documentation.
- Liaise with financial management services concerning enrollments and budget plan revisions, ensuring compliance with federal and state-funded program regulations.
- Generate ad hoc reports as required, providing valuable insights and data-driven recommendations for program improvement.
- Update and submit the VD-HCBS Monthly Update Spreadsheet by established deadlines, promoting transparency and accountability.
- Conduct background checks for employees selected by the veteran for hire, ensuring the safety and well-being of all parties involved.
- Create and maintain a summary page for each veteran, ensuring updates as necessary, and providing a comprehensive overview of their needs and services.
- Provide a detailed home visit/phone call spreadsheet with notes to the Program Manager by the 10th of each month for the previous month's activities, promoting effective communication and collaboration.
- Compile mileage reports using designated software and submit them with supporting documentation by the 7th of each month for the prior month, ensuring accurate reimbursement and accounting.
- Participate in meetings and conference calls with local and national VA representatives, staying informed about the latest developments and best practices in veteran services.
- Adhere to all security and confidentiality regulations, maintaining the trust and confidence of our veterans and stakeholders.
- Perform additional duties as assigned by the Program Manager or senior leadership, demonstrating flexibility and adaptability in a dynamic and ever-changing environment.
