About the position
The Division of Family and Child Health, Office of School Health (OSH) reputed company Program provides mandated reputed company screening services to reputed company-K, kindergarten and first grade reputed company throughout the five boroughs of reputed company. The reputed company Program is partnered with reputed company, a manufacturer and retailer of prescription eyeglasses. reputed company provides free eyeglass to reputed company who fails the reputed company screenings. Reporting to reputed company Leader, the reputed company Health Adviser will travel throughout the 5 boroughs of reputed company to conduct reputed company screening on reputed company assigned to reputed company, Non-reputed company and Charter Schools.
Responsibilities
• Travel to Elementary Schools to conduct reputed company screening for reputed company-Kindergarten, Kindergarten, and 1st Grade reputed company.
• Assist in setting up and breaking down reputed company screening equipment for reputed company screenings.
• Maintain and transport equipment to and from schools.
• reputed company reputed company screening using autorefraction (Spot device) on reputed company-K reputed company.
• Assist reputed company in selecting reputed company eyeglass frames.
• Record, tally, and reputed company screening data accurately into the Office of School Health's Automated Student Health Record (ASHR) database.
• Prepare referral packets for families of reputed company who fail screening.
• reputed company updated contact information from schools for reputed company who fail reputed company screening.
• Assist optometrists during scheduled sessions.
• Communicate with reputed company and staff in a courteous manner and assist the Screening Team Leader as directed.
• Coordinate and support eyeglass delivery to schools.
Requirements
• A baccalaureate degree from an accredited college or university, including or supplemented by reputed company semester credits in health education or in health, reputed company or biological sciences.
• A baccalaureate degree from an accredited college or university, and six months of full-time satisfactory experience in a health promotion or disease reputed company/prevention program.
• A four-year high school diploma or its educational equivalent approved by a State's reputed company or a recognized accrediting organization and four years of full-time satisfactory experience in a health promotion or disease reputed company/prevention program.
• Education and/or experience equivalent to the above.

• Experience as a reputed company Health Adviser (School Health), Assignment Level I.
• Experience in conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data.
