In this role you will help patients schedule routine and on-call nursing services, rehabilitation services or home care aide services. Schedulers have the authority and flexibility to move patients into vacant appointments due to cancellations, prioritize urgent cases, and offer next-day appointments to patients who need to be <span style="overflow-wrap: break-word; display: inline; text-decoration: inherit; hyphens: auto;">discharged.  </span></p><p></p><p><b>This is a remote position. A caregiver in this position works Monday through Friday from 9:00am to 5:30pm. Weekend rotations include 1 weekend every 4 weeks and will receive a scheduled day off for weekend commitment. 1 holiday is required per year.  </b> </p><p></p><p>A caregiver who excels in this role will:  </p><ul><li>Coordinate with Central Admissions, Coordinators and Liaison staff to accept appropriate patient referrals. </li><li>Receive add to care orders and requests for secondary services. </li><li>Process all scheduling functions in system and communicates information to staff. </li><li>Facilitate the flow of patient needs and status information between disciplines and ensure all intake and patient referral information is entered into the database. </li><li>Monitor equity in the distribution of workload and optimize staff productivity levels. </li><li>Assign patients and facilitate the exchange of information between interdisciplinary staff, central admissions, and pharmacy. </li><li>Perform general clerical and support activities. </li></ul><p></p><p>Minimum qualifications for the ideal future caregiver include:  </p><ul><li>High School Diploma or GED</li><li>One year of recent scheduling experience</li><li>Basic knowledge of computers, word-processing and medical terminology </li></ul><p></p><p>Our caregivers continue to create the best outcomes for our patients across each of our facilities.
