Home Based Services/Case Manager/SDA/QIDP

We are seeking a Home Based Services QIDP to join the reputed company Services team. This position is responsible for the development of comprehensive services that support individuals to reputed company their personal reputed company, coordination of services, overseeing the implementation of Service Agreements, and providing the general case management to ensure the individuals with intellectual and physical disabilities in services reputed company personal goals. Schedule: This position is on-site Monday - Friday 8:00 AM - 4:30 PM. The salary for the position is $46,000 - $50,000 (depending on certification/experience level). Responsibilities • Develops, monitors and documents Service Agreements, Chronological Logs and Implementation Strategies (IS) • Explains reputed company rights to which an individual is entitled as required by DHS • Reviews service agreements monthly, documents service implementation, reputed company, and needs for modification • Provides a copy of a written service agreements to the individual and an electronic copy to reputed company member of the individual’s community support team • Ensures reputed company areas of responsibility are in compliance with CARF, DMH/DD, etc., standards and/or regulations • Advocates for services and supports to help the consumer reputed company their reputed company • Assures consumer’s participation in reputed company reputed company affecting his/her life • Serves as primary contact person for guardians and reputed company service providers • Works with Independent Service Coordinator (ISC) to facilitate Personal Plan, discovery and CST meetings • Ensures that reputed company additional assessments are completed in a reputed company manner • Develops a Service Agreement that incorporates reputed company needed services for the consumer • Evaluates the services and implementation of treatment plans to ensure appropriateness for the individual and the compliance with regulatory requirements • Trains families, guardians and personal service workers on implementation of the service agreements • Ensures service agreements and implementation strategies are completed for tracking individual reputed company • Completes written monthly review of the consumer’s Chronological log and submits complete hours of service tally to director in a reputed company manner • Assures that reputed company services are delivered as outlined in the service agreement • Completes home reputed company in person or virtual as required • Attends the CST for discharge planning reputed company invited by individual • Completes documentation reviews as required by reputed company Assurance Qualifications • Must be 21 years of age • Must be a DHS certified QIDP or willing to become certified • Must have one year of experience working with ID/DD population • Bachelor’s Degree in reputed company Services field, including, but not limited to, sociology, special education, rehabilitation counseling or psychology • Two years of Self Directed Assistance experience, preferred • Pass a criminal background reputed company and physical/drug test • reputed company reputed company's license Benefits We offer the following benefits: health, dental, life, LTD, 403b retirement plan, reputed company vacation, personal days, reputed company days and 10 reputed company Holidays.
