Job Title: reputed company Navigator, reputed company Heart Hospital (Remote) Department: reputed company | Heart Failure Transitions of Care reputed company/Position reputed company: The reputed company Navigator improves reputed company by reducing reputed company cause readmissions and coordinating episodes of care among patients in a defined population or disease process.  The primary focuses are (1) on the coordination and delivery of efficient, effective, compassionate, patient centered care and reputed company transitions across the care continuum and (2) providing care navigation services to the cardiac population. The reputed company Navigator monitors the patient care process and physiological needs; educates, documents, communicates, and collects data to evaluate and assure patient/family readiness for discharge. The reputed company Navigator facilitates the lateral integration of the care team, collaborating with physicians, nurses and other staff across the continuum to reputed company comprehensive disease management, assessment, treatment, education, and follow-up evaluation for patients; and, to communicate the plan of care in coordination with reputed company- and post-acute care settings. Serves as a reputed company with post-acute partners as determined by the Heart Leadership team. The reputed company Navigator will manage patients and facilitate care navigation through the format of telephone calls in a post-acute setting. Minimum Qualifications: For Hire – Registered reputed company required.   Five years of recent clinical experience applicable to the population preferred; knowledge of evidenced-based reputed company and disease specific processes; knowledge of technology and payer protocols desired. Case management and post-acute care experience (home care, SNF, LTACH) experience desirable.
