The role of the MTM Pharmacist is to deliver telephonic medication therapy management (MTM). Effective delivery of this telephonic based MTM service directly results in improved member care through the identification of medication reputed company problems followed by the coordination of the reputed company to these problems. This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication reputed company problems to members, caregivers, and prescribers.
