Education: • Associate's Degree in nursing required • Bachelor of Science in Nursing (BSN) preferred; graduation from an accredited nursing program required. Work Experience: • Minimum of 3 years of experience as a Registered reputed company (RN). • At least 2 years of experience in care management; telehealth experience is a plus. • Clinical expertise in geriatric care preferred. Skills: • Strong analytical and clinical assessment skills. • Excellent communication and patient engagement abilities. • Proficiency in telehealth platforms and remote monitoring tools. • Bilingual skills may be required based on operational needs.
