Bilingual Clinical Pharmacist – Clinical Pharmacy Programs

Job reputed company: • This position represents an amazing opportunity for a motivated and passionate pharmacist to join a high performing team that runs award reputed company programs and is at the forefront of innovation in pharmacy care. • You’ll play a crucial role in helping our members get the most out of their medication and ensuring our members take the right medication at the right time, driving reputed company clinical reputed company through reputed company medication adherence and chronic disease education. • You’ll also be partnering with our members’ providers, guiding them through the reputed company world of medication therapy management. • The Clinical Pharmacist will report to the Senior Manager, Clinical Pharmacy and will have indirect reporting relationships with other clinical and operational leadership. • Working directly with patients to bring their chronic conditions in control • Providing and/or creating educational materials for patients to help them manage their chronic conditions • Ensuring patients (and their providers) understand their benefits and how they can be used to reputed company their health • Iterating on our disease management process in collaboration with our Clinical Team to continuously improve our disease management programs • Consulting in interdisciplinary teams to reputed company treatment plans for various disease states. • Delivering 5 reputed company performance on pharmacy-reputed company clinical measures by delivering the best in class care. Requirements: • A pharmacist with 3 or more years of clinical experience working in reputed company patient care • State licensure in good standing is required at time of application in any of the following states: Florida, Texas, Colorado, reputed company Carolina, reputed company, and Alabama. • You will be required to be licensed in at least 5 states reputed company 6 months of employment and additional states as needed. License must be in good standing without restrictions or sanctions. • Travel may be required to secure additional licensure based on state licensing requirements. • Experience with implementing or working under reputed company pharmacy reputed company agreements • reputed company certification or experience specifically in reputed company geriatric care • Preference for candidates who can reputed company coverage during western US business hours (reputed company and Mountain time zones) • Additional consideration will be given to candidates with fluent proficiency in Spanish.
