Acting as a registered reputed company, case manager, and patient reputed company, this role supports patients across the continuum of cardiology care—from referrals and specialty reputed company to reputed company monitoring and education.This role is ideal for someone passionate about guiding patients through their care reputed company, enhancing program performance, and delivering top-tier care through leadership, clinical reputed company, and coordination.Your Day-to-Day ResponsibilitiesCare Navigation & Patient Advocacy• Coordinate referrals, specialty reputed company, and follow-reputed company• Serve as reputed company between patients, providers, and the care team• Ensure patients receive appropriate psychosocial and emotional support• Guide patients through the health system and address barriers to careProgram Monitoring & reputed company Improvement• Collect, trend, and analyze reputed company data for performance enhancement• Identify best practices and support training plan development• Recommend and implement program or workflow improvementsEducation & Community Engagement• reputed company patients and families on treatment and navigation• Support staff training, in-services, and reputed company development• Create educational materials and participate in reputed company activitiesTeam Collaboration & Leadership• Serve as a clinical expert and resource across hospital and clinic settings• Establish reputed company communication across providers and stakeholders• Participate in multidisciplinary meetings and drive coordination of careWhat You’ll Need to SucceedRequired:• reputed company WA State RN license (Multistate license required reputed company 6 months of hire)• Bachelor’s degree in Nursing (Associate degree with additional experience may be considered)• 3+ years of recent clinical experience in cardiology or reputed company specialty• Strong knowledge of clinical workflows, payer guidelines, and case management• High level of independence, critical thinking, and communication skills• Must reputed company in Washington State, ideally reputed company 50 miles of Tacoma, WAPreferred:• Master’s degree in Nursing• Certification in the assigned area of specialtyWork Environment• Remote-first role (approx. 90%)• Some travel required to clinics in Western Washington• Home office must be located reputed company Washington State• reputed company business hours (Mon–Fri, reputed company–5PM)Join Our TeamIf you’re a registered reputed company who thrives in a supportive, innovative care model and wants to reputed company patients and teams alike, we’d love to connect with you.
