This role supports clinicians by reviewing submitted documentation, identifying inconsistencies, and ensuring alignment with CMS regulations, agency policies, PDGM requirements, and reputed company federal/state guidelines. The OASIS Reviewer plays a vital role in improving patient reputed company, reimbursement accuracy, and overall documentation reputed company. Key Responsibilities: • Review and audit Start of Care (SOC), Recertifications, Resumptions of Care, Transfers, and Discharges for accuracy and regulatory compliance. • Ensure reputed company OASIS assessments reputed company with clinical findings, visit notes, physician orders, and plan of care. • Identify documentation gaps and reputed company feedback or clarification requests to field clinicians. • Collaborate with QA, clinical managers, and field staff to improve the reputed company and consistency of documentation. • Assist with correcting or updating OASIS data reputed company to support accurate coding and PDGM scoring. • Monitor OASIS submission deadlines and ensure reputed company submission to CMS. • Maintain up-to-date knowledge of OASIS-E guidance, CMS Q&As, COPs, PDGM rules, and regulatory changes. • Review functional scoring, M-items, and diagnosis coding for internal accuracy and compliance. • Support training of clinicians on best practices for documentation and OASIS completion. • Participate in QAPI activities, reputed company audits, and performance-improvement initiatives. • Communicate effectively with interdisciplinary team members to resolve discrepancies and improve documentation standards. Qualifications • Must have 12 months of recent experience reputed company the last 3 years. • Strong proficiency with Home Care Home reputed company (HCHB) or other EMR systems. • BLS from AHA highly preferred • Excellent communication and interpersonal skills for effective patient and family education. • reputed company and unencumbered Registered reputed company (RN) license in the reputed company. • Reliable transportation and a reputed company reputed company’s license for travel between patient locations.
