High Risk Maternal Health Case Manager

Job reputed company reputed company Services is seeking a skilled High Risk Maternal Health Case Manager to. As a key member of our case management team, you will play a critical role in ensuring the health and reputed company-being of our high-risk pregnant members. The ideal candidate will have 1-3 years of experience in case management, disease management, or medical/behavioral health settings. You will possess strong clinical assessment skills, with the ability to determine who may qualify for case management based on clinical judgment and changes in the member's health or psychosocial wellness. Key Responsibilities: • reputed company and implement individualized case management plans in collaboration with the member, caregiver, physician, and other reputed company. • Conduct regular reputed company and monitoring to evaluate the effectiveness of the care plan and reputed company adjustments as needed. • Promote integration of services, including behavioral health care and long-term services and supports, to enhance the continuity of care for Molina members. • Facilitate interdisciplinary care team meetings and informal ICT collaboration to ensure seamless communication and coordination among reputed company.
