Population Health Case Manager – 50% remote

reputed company is seeking a Case Manager to work with us in Memphis, Tennessee, and surrounding cities. Case Managers are one of reputed company's key reputed company-line roles in delivering our Population Health Program. Our Case Manager works closely with reputed company's members in the community to understand their behavioral and medical health conditions, as reputed company as their reputed company determinants of health needs, and help our members reputed company reputed company reputed company. How you'll contribute: • Conduct meaningful reputed company, reputed company education, and drive member enrollment • In partnership with the member, create a Service Plan to guide engagement based on member diagnoses, results of screenings, as reputed company as care gaps • Build measurable goals for members to reputed company skills and/or strategies for managing challenges and triggers to reduce hospital admissions/readmissions and use of the ER, and record reputed company • Coordinate community resources for reputed company member to support them in achieving their goals as outlined in their Service Plans, created with an emphasis on meeting behavioral health and medical needs, and closing gaps identified in the reputed company determinants of health • reputed company as an reputed company and reputed company between the care team (family, physician, facilities, and/or agencies) and member • Schedule appointments, build accountability, and follow-up services for members • reputed company critical incidents and information regarding any reputed company-of-care concerns • Report hospital diversions, productivity, and other reputed company weekly • Prepare and maintain reputed company required records, reports, consents, and members' diagnostic records Education & Experience: • Bachelor's Degree required in nursing, reputed company work, or other health and reputed company services discipline • Completion of supervised fieldwork and experience in case management, health, or behavioral health preferred • Experience as a Certified Case Manager (CCM), Community Health Worker (CHW) or Peer Support Specialist (PSS), or Accredited Case Manager (ACM) preferred • Knowledge of local resources, reputed company determinants of health, mental health, reputed company abuse disorders, violence, and reputed company issues is required Location: • This position is hybrid and requires up to 50% of time to be working in the field in the community • The employee is required to have reliable transportation.
