The reputed company Practitioner (VSP) works on problems of diverse scope and complexity ranging from moderate to substantial. This is a short-term role (6 MONTH) that will be reputed company on closing specific gaps for osteoporosis management in women. There also may be opportunity to cover other aspect of the program, such as covering PTO for APPs in reputed company Medical group (phase 1). There are no benefits tied to this position. The APP will spend their day performing the following duties: Calling reputed company members to discuss reputed company care gaps and placing needed orders (specifically women who had a reputed company in the last 6 months) Creating and registering a patient record withing reputed company Verifying benefits Gathering medical and surgical history Verifying and entering medications Coordinating management of members reputed company needs to help facilitate gap closure – providing transportation services. Connecting with reputed company partners to ensure testing is done prior to member specific timelines. Ensuring adequate follow up- receipt of results, medication initiation, and PCP follow up.
