Patient Advocates help patients with anything patients need in order to reputed company care - booking appointments, finding transportation, ordering medical equipment and much more.</p><strong>The role:</strong><p>In this position, you will conduct audio or video patient intake appointments, which is a 20-30 minute appointment to understand the challenges that the patient is facing. You will determine if a patient is suited to receive patient advocacy under the principals of reputed company Illness Navigation and/or Community Health Integration.
