<p><span data-contrast="auto">In the role of an Advanced Care Provider (Physician Assistant or Nurse Practitioner) on our Virtual Care team, your role as a concierge clinician is to be there for patients in a members-only environment, when they need us most. </span><span data-ccp-props="{"134233117":false,"134233118":false,"201341983":0,"335551550":1,"335551620":1,"335559685":0,"335559737":0,"335559738":0,"335559739":160,"335559740":276}"> </span></p> <p><span data-contrast="auto">You will work rotating shifts on our Aftercare and Triage services providing guided care to our members. </span><span data-contrast="auto">Our Aftercare team is responsible for post-visit follow up medical care and management for patients seen in our clinics, on a house call or via telemedicine that needed urgent care. Triage</span></strong><span data-ccp-props="{"201341983":0,"335559740":276}"> </span></p> <ul> <li><span data-contrast="auto">Triage incoming patients needs, assess their concerns and determine the best course of action including where their care should best be delivered - a Sollis center, virtually or via a requested house call visit. </span><span data-ccp-props="{}"> </span></li> <li><span data-contrast="auto">Review medical inquiries from Sollis patients that arrive via email, text or phone, monitor navigator tasks and respond in a timely manner. Graduate of an accredited PA or NP program, required.
