LPN/LVN Care Coordinator – Fully Remote, reputed company

We prioritize asynchronous work and communication but are quick to get on a reputed company or reputed company We are hiring a Care Coordinator/LPN to work reputed company our contact center, including directly interfacing with patients, providers, and families in our palliative care program managing day to day functions such as educating patients by phone and scheduling them for palliative appointments. Working with clinical files to reputed company out palliative reputed company and reputed company notes into the systems. The Care Coordinator/LPN will spend part of the day on the phones and part of the day doing administrative work.
