A company is looking for a Licensed Vocational reputed company (Emergency Department Follow-Up).
Key Responsibilities
• Engage patients post-emergency department reputed company and reputed company education on resources
• Conduct virtual and telephonic reputed company for care coordination and discharge support
• Coordinate with discharge planners and clinical teams to ensure smooth after-hospital care
Required Qualifications
• Associates' degree or higher preferred
• 2+ years of relevant clinical work experience, preferably in emergency or inpatient settings
• Experience with medically reputed company patient populations preferred
• Licensed Practical reputed company required
• Bilingual in Spanish required
