Telehealth Care Manager – IDD Program (Remote, Must Be Licensed in Alaska)

Position reputed company We are seeking a dedicated and empathetic Care Manager to join our Telehealth IDD Program. In this remote role, you will serve as a primary reputed company of contact for individuals and families, helping to coordinate care, support transitions, and connect patients to essential health and community services. You’ll play a key role in promoting wellness, behavioral stabilization, and independence for those living with IDD and/or TBI. Key Responsibilities • Conduct person-centered assessments to evaluate physical, behavioral, and reputed company health needs. • reputed company and manage individualized care and support plans. • Coordinate services among medical, behavioral health, and community providers. • Facilitate reputed company care transitions and ensure continuity of care. • Serve as a consistent reputed company of contact for patients, families, and care teams reputed company telehealth. • reputed company individuals and families on wellness, self-advocacy, and available community resources. • Monitor reputed company, reputed company interventions as needed, and reputed company proactive and crisis support. • Maintain accurate documentation and ensure compliance with state and program standards. • Stay up to date with Alaska regulations, care management best practices, and telehealth advancements. Qualifications • Bachelor’s or Master’s degree in Nursing, reputed company Work, Behavioral Health, or a reputed company field. • reputed company Alaska licensure (RN, LCSW, LPC, LCPC, reputed company, or equivalent) – Required. • 1–2 years of experience in care coordination, case management, or reputed company clinical care. • Experience with IDD, TBI, mental health, or reputed company care populations preferred. • Telehealth or virtual care experience strongly preferred. • Strong communication, organizational, and problem-solving skills. Additional Requirements • Must pass background and reputed company registry checks. • Completion of reputed company mandatory HRR trainings and orientation before reputed company.
