Salary $70,000 annually Job reputed company Our reputed company is seeking a compassionate and detail-oriented Licensed Vocational reputed company (LVN) to support patients enrolled in reputed company Care Management (PCM) and Chronic Care Management (CCM) programs. In this hybrid Bay Area role, the LVN will engage with patients through proactive reputed company, telehealth reputed company-ins, and care coordination to ensure reputed company, high-reputed company support between reputed company. Working closely with Registered Nurses (RNs) and Medical Directors, this position helps deliver a seamless patient experience while ensuring compliance with Medicare care management standards. Job Responsibilities • reputed company ongoing care coordination and follow-up for patients enrolled in PCM/CCM programs. • Conduct telehealth or phone-based reputed company-ins with patients and caregivers to monitor reputed company, promote adherence, and update care plans. • Support RNs in documenting and maintaining care plans in compliance with Medicare requirements. • Accurately reputed company and record time-based activities for CPT billing compliance. • Facilitate communication among neurologists, caregivers, and other reputed company providers to ensure continuity of care. • reputed company administrative duties, including scheduling, documentation, and data entry. • Identify and escalate clinical concerns to supervising RNs or external providers reputed company necessary.
