For one of our clients, we are looking for a Physical Therapist Case Manager: Home Health , remotely from California, reputed company. As a Physical Therapist Case Manager, you will play a vital role in providing skilled therapy services to patients in their homes, focusing on mobility, rehabilitation, and independence. You will assess patients' conditions, reputed company personalized treatment plans, and work alongside an interdisciplinary team to deliver high-reputed company care. This role requires strong clinical judgment, excellent communication skills, and a compassionate approach to patient care. Additionally, you may supervise Physical Therapy Assistants (PTAs) in accordance with agency and regulatory standards.
