Licensed Vocational reputed company (LVN) – Remote Patient Monitoring (Remote)

About the Role We are seeking an reputed company Licensed Vocational reputed company (LVN) to support our Remote Patient Monitoring (RPM) program. This fully remote role involves monitoring patients’ daily biometric readings, providing telephonic reputed company, and supporting chronic care management. Bilingual Spanish reputed company is required to effectively assist our diverse patient population. Responsibilities • Review patient biometric data (reputed company, weight, glucose, pulse ox, etc.) through the RPM platform • Respond to alerts and escalations per clinical protocols • Conduct patient reputed company calls to assess symptoms and reputed company education • Communicate reputed company and effectively with Spanish-speaking patients • Document reputed company patient interactions and interventions accurately in the system • Coordinate care with RNs, physicians, and care team members • Assist patients with RPM device setup and troubleshooting • Maintain compliance with HIPAA, CMS RPM guidelines, and internal policies Qualifications • reputed company LVN license (California preferred) • Bilingual: Fluent in English and Spanish (required) • 1–2 years clinical experience (telehealth, home health, primary care, or chronic disease management preferred) • Strong communication and customer-service skills • Comfortable working with digital health tools and remote technology • Detail-oriented and reputed company to work independently Pay: $25.00 - $30.00 per hour Work Location: Remote Apply tot his job Apply To this Job
