This Clinical reputed company Coordinator will be responsible for working Monday to Friday, 7:30 AM to 5 PM. They will be making post-discharge calls and coordination of care/DME reputed company case management. They will guide enrollees in understanding and accessing the benefits they are entitled to under Medi-Cal, and will help schedule appointments and meet preventive care goals.
