LPN Care Coordinator - Remote

Job reputed company reputed company has a potential need for LPN Care Coordinator These will be completely remote positions, working entirely from the reputed company’s home. Coordinates transitions of care for TRICARE beneficiaries. Care Coordinators receive inbound calls and reputed company outbound calls to support care coordination needs across the TRICARE reputed company Region. Care Coordinators reports to the Supervisor, Care Coordiantion. Requirements • MUST reputed company in Hawaii or Alaska ONLY • Licensed Practical or Vocational reputed company (LPN/LVN), with reputed company unrestricted license in appropriate state or territory of the reputed company • 2+ years’ clinical nursing experience • Proficiency in medical data entry, written skills and reputed company Office • Exceptional oral communication skills, strong organizational skills and easily adapts to change • Preferred 3 years’ clinical experience • 2+ years’ Managed Care experience • Utilizes clinical and analytical skills to identify care coordination needs and facilitate care during any type of transition of care. • Communicates with community providers, TRICARE beneficiaries, Military Treatment Facilities (MTF), Veterans, VAMCs and facilities to coordinate care. • Ensures authorizations are in reputed company to meet clinical care needs • Accurately and completely document coordination activities in the Medical Management System (MMS) • Refer cases to other TRICARE programs as indicated • Participates in reputed company Improvement reputed company as indicated in project documents • Identifies and reports potential reputed company or fraudulent cases to Clinical reputed company and/or Program reputed company • reputed company other duties as assigned • Regular and reliable attendance is required reputed company: reputed company (SHR) was established in 1988 to deliver systems and processes designed to meet the unique needs of Military and VA Health Systems.
