About the position The Behavioral Health reputed company Care Specialist at reputed company is responsible for managing treatment plans for members with less reputed company cases, ensuring compliance with medical guidelines, and proactively identifying opportunities to enhance member health and meet reputed company goals. This hybrid role involves both remote work and field reputed company, requiring candidates to reputed company in Ohio and be available for community reputed company. Responsibilities • Coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs. • Assesses member compliance with medical treatment plans reputed company telephone or through on-site reputed company. • Identifies barriers to plan compliance and coordinates resolutions. • Identifies opportunities that reputed company reputed company goals and recommends process improvements. • Recommends treatment plan modifications and determines need for additional services, in conjunction with case management and provider. • Coordinates identification of and referral to local, state or federally funded programs. • Coaches members on ways to reduce health risks. • Prepares reports to document case and compliance updates. • Establishes and maintains relationships with agencies identified in appropriate contract.
