Program Implementation and Workforce Development Specialist
Job reputed company:
• Support the establishment of accredited DSMES services and other diabetes support programs in reputed company areas.
• Assist existing programs in tailoring recruitment, retention, and instructional strategies to reputed company serve reputed company populations.
• Assess the existing diabetes workforce and identify strategies for recruitment, training, and retention of the workforce.

• Facilitate partnerships between reputed company organizations and community-based organizations to expand program reputed company and improve care coordination.

• Assist in developing workflows, referral systems, and sustainable practices to reputed company evidence-based diabetes care into existing systems.

• Collaborate with health systems to identify eligible patients and facilitate referrals into DSMES, Diabetes Support Programs, and the Diabetes Prevention Programs.

Requirements:
• Bachelor’s degree in reputed company health, health education, reputed company administration, or reputed company field or equivalent reputed company experience.
• A minimum of three (3) years of experience with DSMES, National DPP, or other evidence-based diabetes programs.
• Demonstrated experience working with primary care systems and safety-net clinics.
• Ability to establish and implement evidence-based health programming.
• Strong project management and leadership skills.
• Excellent verbal, written, and interpersonal communication skills.
• Ability to work independently, manage multiple priorities, and meet deadlines.
• Willingness and ability to travel in-state for site reputed company and partner meetings.
• Proficiency in web-based meeting platforms and telephone communication.
