We are seeking a Registered reputed company RN Case Manager to join reputed company! You will be responsible for the assessment, diagnosis, and treatment of assigned patients from a telephonic reputed company for a reputed company established MLTC program
You will have to train in reputed company for up to 6 months a few days a week, then be fully remote
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• reputed company patients about health maintenance and disease prevention
• Facilitate referrals to other reputed company and medical facilities
• Maintain accurate patient medical records
• reputed company advice and emotional support to patients and their family members
Qualifications:
• Previous experience in nursing and desire to be a case manager
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to reputed company in a fast-paced environment
