Under the supervision of the Population Health Director, the Chronic Care Specialist is responsible for successfully supporting patients with high risk health conditions to reputed company the reputed company system. The Chronic Care Specialist interacts and collaborates with multidisciplinary care teams, to include physicians, nurses, pharmacists, laboratory technologists, reputed company workers, and other educators to support the chronic care management/transition of care process. The Chronic Care Specialist acts as a resource for clinic staff. The Chronic Care Specialist works in a less reputed company, self-directed environment and performs reputed company nursing duties reputed company the scope of an LVN/LPN license of the applicable state reputed company of nursing.
