**Job Summary**
We're seeking an experienced remote social worker to join our team. As a licensed Masters Social Worker, you'll work with a registered nurse case manager to research available options and resources for members with financial, transportation, safety, and housing needs. Your population will span the entire lifecycle, and all member contact will be done telephonically or via electronic exchange. You'll empower members to make positive lifestyle and medical choices and work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
**Responsibilities**
* Research available options and resources for members with financial, transportation, safety, and housing needs
* Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet members' benefit plan and/or health needs
* Empower members to make positive lifestyle and medical choices
* Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs
* Collaborate with other healthcare professionals to ensure comprehensive care for members
**Requirements**
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
* 2+ years of experience in medical social work or case management
* Masters Degree in Social Work
**Preferred Qualifications**
* Experience working with diverse populations, including children, adults, and older adults
* Knowledge of community resources and services, including financial assistance programs, transportation services, and housing options
* Strong communication and interpersonal skills, with the ability to work effectively with members, healthcare professionals, and other stakeholders
* Ability to work independently and as part of a team
* Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions
