Oncology Nurse - Transitions of Care
A company is looking for an Oncology Transitions of Care Nurse.
Key Responsibilities
• Conduct clinical assessments and monitor changes in health for members diagnosed with cancer
• Coordinate care transitions and educate members and caregivers about diagnosis and treatment
• Assist with administrative projects and communicate feedback to improve service offerings
Required Qualifications
• Bachelor of Science in Nursing (BSN) and a compact unrestricted Registered Nurse (RN) license
• Minimum of 5 years of nursing experience, including 3 years in oncology nursing or case management
• Certified Case Manager (CCM) certification required
• Ability to work in a fast-paced environment and manage multiple tasks effectively
• Willingness to learn and use new technologies, including electronic health records and communication tools
