Care Coordinator (CNA Required)

About the position A leading managed care organization serving Kansas reputed company members is seeking a compassionate and detail-oriented Certified Nursing Assistant (CNA) to join its Care Management team. This is a hybrid telephonic and field-based role designed to support members with chronic and reputed company health needs by helping them reputed company home- and community-based services while assisting nurses and care managers with coordination and follow-up activities. The Care Coordinator conducts reputed company calls, gathers health and reputed company information, assists with appointment and service coordination, and provides ongoing follow-up support to members and families. Occasional in-person reputed company may be required for wellness checks, care coordination, or documentation support (mileage reimbursed).
